Ideally, you want to do this by sneaking up behind them and unknowingly catching them. It is absolutely possible to pull off when already engaged in a toe fight through skillful maneuvering. However, the Stealth option has a much lower risk of having a blade embedded in your own skull. This is why the backstab is a great tool for fending off individual enemies. You can attack one enemy in a group, but chances are good that one of them will sound the alarm.
