Focus on items that increase your base stats: Strength, Intelligence, Fortitude, Vitality, and Willpower.
Upgrade your gear after following all the instructions mentioned above. Set, Rare and Legendary items that get upgrades give additional boosts to the aforementioned stats, thereby increasing your combat rating. Also, be sure to socket Gems, especially Legendary Gems, some of which can directly raise your rating.
